During the Meredith Era of Millionaire, contestant Gordon Purcell had the following question to answer:
"How is the "S.O.S." distress call expressed in Morse code?" A: 3 dashes, 3 dots, 3 dashes B: 3 dots, 3 dashes, 3 dots C: Dash, 2 dots, dash D: Dot, 2 dashes, dot
This exact same question appeared again years later for celebrity contestant Ike Barinholtz, on the 20th Anniversary version.
Whether this was intentional or by accident, I do not know. I'm inclined to believe it was intentional, as the second appearance of the question was for charity. Coincidentally, however, each contestant used phone-a-friend for help. One difference between the two is that the question was initially worth $50,000, and later valued at $125,000.
Anyways, I thought this was an interesting find and wanted to share.

The same thing happened over here in the Philippines!
On this 2002 Valentine's Day episode with Christopher De Leon, the couple playing was asked about the location of Hospicio San Jose, a foster care institution has been a home for orphans, the abandoned, special needs, and the elderly. The answer was erroneously written as "Isla de Convalencia" with the right answer being "Isla de Convalecencia" ("Isla de la Convalecencia is also a correct name), a small island within the Pasig River:
And the question was featured again in Sharon Cuneta's run on Millionaire with Vic Sotto in 2010 as she chose the hospice to be the charity to donate her winnings to:

A lot of questions during the 20th Anniversary run were old questions reused.
